Procedure for Lost or Damaged Shipment
Although, we take all reasonable precautions to ensure your orders will be delivered on time and in good condition, we have little control over the postal process and the loss or damage of an order may occationally occur.

At Paradoxal Press, we insure all our packages and will gladly send you a replacement copy of your order if it meets the following criteria
  • Order not recieved after 21 days for USA domestic orders and 28 days for international orders.
  • Your order was received on-time but arrived in a damaged condition.

In such cases, you are entiteled to a replacement or refund of the lost or damaged items. To complete such a request, simply fill the "Statement of Lost or Damaged Shipment" below by filling the necessary information and signing the document elecronically at the bottom of the page.

Once filled, we will process your request within 24 hours and issue a credit or replacement as requested.

Affidavit For Lost or Damaged Parcels
I hereby certify that all information on this form is accurate and truthful. The submission of a false, fictitious or fraudulent statement may result in imprisonment of up to 5 years and a fine of up to $10,000.00 (18 USC 1001). In addition, a civil penalty of up to $5,000.00, and an assessment of twice the amount falsely claimed may be imposed (31 USC 3802).

WARNING: ANY FRAUDULENT CLAIMS WILL MAKE THE SHIPPER AND/OR CONSIGNEE LIABLE FOR ANY PROSECUTION FOR MAIL FRAUD UNDER FEDERAL CRIME CODE.
